Civic QA remains self-service and unofficial; publication transfers no authority and is not operational deployment, efficacy, safety, certification, a legal-compliance finding, permission to act, or a global green.

A finding is not a hot take or a score alone. It ties an explicit criterion to evidence, consequence, recurrence mechanism, accountable ownership, corrective action, and a verifiable definition of closed.
Use the highest material state supported by evidence. Name uncertainty. Do not inflate severity for rhetoric or lower it to protect an institution.

A systemic deviation capable of materially changing access, rights, burden, reliability, or consequential outcomes.
A bounded deviation that weakens quality or accountability but has a contained consequence and usable recovery.

The action deadline, interim safety condition, escalation trigger, and condition that blocks closure.
The prewritten effectiveness threshold, independent reviewer, evidence window, and publication receipt.
Implemented is not effective. Trained is not effective. Published is not effective. Closed means the defined result occurred, the burden was checked, recurrence is controlled, and the receipt is public.
