Project Shadow 1.0.1 contains no Myth package. Generic Myth v0.2.0 and Full-Canon Myth v0.3.5 are separate optional companions; both default off, neither is required by R1, and neither can authorize action or change an R1 result. Civic QA remains self-service and unofficial; publication transfers no authority and is not operational deployment, efficacy, safety, certification, a legal-compliance finding, permission to act, or a global green.

QA-03Findings + severity

Write the deviation so repair cannot escape through vagueness.

A finding is not a hot take or a score alone. It ties an explicit criterion to evidence, consequence, recurrence mechanism, accountable ownership, corrective action, and a verifiable definition of closed.

FIVE FINDING STATES

Severity follows consequence and control—not embarrassment.

Use the highest material state supported by evidence. Name uncertainty. Do not inflate severity for rhetoric or lower it to protect an institution.

Red

CRITICAL

Active or imminent severe harm; unlawful or unbounded authority; closed recourse; or a control failure requiring immediate stop.

Orange

MAJOR

A systemic deviation capable of materially changing access, rights, burden, reliability, or consequential outcomes.

Yellow

MINOR

A bounded deviation that weakens quality or accountability but has a contained consequence and usable recovery.

Blue

OBSERVATION

A condition not yet a deviation but important to monitor, clarify, or test before the risk grows.

Green

GOOD CONTROL

A safeguard with evidence that it works; preserve it, test drift, and do not generalize beyond its coverage.

ANATOMY OF A COMPLETE FINDING

Eight fields make the path reconstructable.

If a field is unknown, leave it visibly unknown and assign the next evidence action. Do not manufacture completeness.

    CONDITION

    The specific observed or verified state that deviates from a standard.

  2. 02

    CRITERION

    The explicit law, policy, promise, control, specification, or quality standard used for comparison.

  3. 03

    EVIDENCE

    Sources, coverage, dates, methods, unknowns, and strongest competing explanation.

  4. 04

    CONSEQUENCE

    Who is affected, how, at what scale, with what power to refuse, appeal, exit, or recover.

  5. 05

    MECHANISM

    The authority, incentive, workflow, data, dependency, interface, or failed control that enables recurrence.

  6. 06

    OWNER

    The role capable of correcting the present condition and the role responsible for prevention.

  7. 07

    DUE + GATE

    The action deadline, interim safety condition, escalation trigger, and condition that blocks closure.

  8. 08

    VERIFICATION

    The prewritten effectiveness threshold, independent reviewer, evidence window, and publication receipt.

REQUIRED OUTPUTA skeptical reader can reproduce the finding, identify the owner, challenge the frame, and test whether the repair earned closure.
CLOSURE RULE
Implemented is not effective. Trained is not effective. Published is not effective. Closed means the defined result occurred, the burden was checked, recurrence is controlled, and the receipt is public.
